    The Solutions for Out-of-School Youth (SOSY) consortium is collecting staff ratings of the products, materials, and training provided through the consortium.     Student Services PlanWord doc
    Each state participating in the SOSY project is required to complete the OSY Student Profile sheet on as many OSY students as possible. The minimum data elements are contained on the OSY Profile but states are free to add additional data elements if desired. This form, in English and Spanish, can be adapted by the State MEP.  The OSY Student Profile sheet is used to gather essential data at both the individual student and state levels.  At the individual level, the OSY Profile is the starting point for planning the services that best match the needs and availability of each youth identified.  At the state level, the OSY Profile helps gather information about the OSY population in the state which aids in the planning and implementation of programs, allocation of funds, and coordination with other relevant service providers.     SOSY Student Services Plan is an optional form developed as a resource for states.
    The SOSY Student Services Plan may be used by states to help identify goals and resources to support OSY and plan for service delivery. The form can be adapted as needed by State MEP.

  8. SOSY Evaluation of ServicesWord doc (6/2/2010)
    SOSY Evaluation of Services is an optional form developed as a resource for states. It has two purposes: (1) to collect individual student’s evaluation related to service delivery, and (2) to be used as a data collection instrument in developing a statewide evaluation of services delivered to the OSY population. The form, in English and Spanish, can be adapted as needed by State MEP.
